If the painful spot is on the surface, use your finger to put on just enough ointment to cover.
If you need to apply ointment deeper, attach the plastic nozzle and cap (which come with the package) to the tube. Then, remove the cap right before you use the tube and carefully push the nozzle all the way into the rectum and squeeze the tube from the bottom while pulling it out.
After using the nozzle, wash it with warm, soapy water and put it back together.
Note that if the area is very red and painful, you should apply the ointment with your finger instead of the nozzle at first.
