• Vitex agnus-castus (Chaste tree) has a long history of traditional
use in Western herbal medicine (WHM) for the relief of menstrual
irregularities and symptoms of premenstrual tension (PMT).
• The dried fruit from the Vitex plant contains several active constituents
including flavonoids, iridoids, alkaloids and volatile oils.
• The mode of action of Vitex is multifaceted. Vitex acts on dopamine
(D2) receptors in the anterior pituitary gland, decreasing high
prolactin levels. Prolactin is a hormone that primarily functions to help
women produce milk after childbirth. However, in non-breastfeeding
mothers, elevated prolactin can block the production of other
hormones in the body, including oestrogen and progesterone.
Elevated prolactin levels are associated with menstrual irregularities
and symptoms of PMT1
.
• Low doses of Vitex have been shown to increase luteinising hormone
(LH) while decreasing follicle-stimulating hormone (FSH), working
indirectly on achieving oestrogen and progesterone balance within
the body. Oestrogen dominance in females is linked to mood
disorders, including PMS, and menstrual irregularities.
• It is recommended Vitex be taken in the morning, before breakfast, as
this is the time the pituitary gland is the most receptive.

DRUG INTERACTIONS
• Contraindicated with fertility drugs or IVF – the addition of Vitex could
potentially cause hyperstimulation of the ovaries.
• Caution with hormone replacement therapy, oral contraceptive pills
and progesterone drugs – given Vitex’s hormone modulating effect, it
may theoretically interfere with the action of these drugs.
CAUTIONS
• Avoid use in individuals with tumours sensitive to oestrogen or
progesterone.
• Vitex is not generally recommended in females under 20 years of
age as the natural hypothalamic-pituitary-ovarian axis can be easily
disrupted.
• Vitex is most effective during the first three to six months of use. After
that, its effect on the pituitary starts to diminish. It is recommended to
take a break from Vitex for two months before resuming use.
• Not recommended during pregnancy without medical advice and
breastfeeding due to insufficient studies relating to safety.
SIDE EFFECTS
• In some cases, Vitex may initially worsen PMS symptoms.
